A comprehensive history and physical will be done at each well child visit (including growth measurements), anticipatory guidance will be given, and all parental questions will be addressed.
In addition, some additional things will take place at certain routine visits as outlined below.

Newborn: Obtain birth history, review family and social history.

2 weeks:

1 month:

2 months: Pentacel #1 (DTaP/HIB/Polio), Hepatitis B#1 (or #2 if given at birth), Pneumococcal #1, and Rotavirus #1 vaccines

3 months:

4 months: Pentacel#2 (DTaP/HIB/Polio), Hepatitis B#2 (if not given at birth), Pneumococcal #2, and Rotavirus #2 vaccines

5 months: Discuss starting solids

6 months: Pentacel #3(DTaP/HIB/Polio), Hepatitis B#3, Pneumococcal #3, and Rotavirus #3 vaccines

9 months: bloodtest to screen for anemia and lead levels

12 months: MMR #1, Varicela #1, and HIB #4 vaccines and developmental screen

15 months: DTaP #4 and Pneumococcal #4 vaccines

18 months: Hepatitis A #1 vaccine and developmental screen

2 years: Hepatitis A#2 vaccine, bloodtest to screen for anemia and lead levels and developmental screen

2.5 years:

3 years: lead screening, hearing and vision screen

4 years: MMR #2 and Varicela #2 (as Proquad) and DTaP#5 and Polio #4 (as Kinrix) vaccines, lead screening, hearing and vision screen

5 years: lead screening, hearing and vision screen, urine screen

6 years: lead screening, hearing and vision screen, urine screen

Annually from 7yrs-9yrs: hearing and vision screening, urine screen

10 years: bloodtest to screen cholesterol levels, hearing and vision screening, urine screen

11 years: Tdap and Meningococcal #1 vaccines, hearing and vision screening, urine screen

12 years: start HPV vaccine series (requires 3 vaccines in 6 months), hearing and vision screening, urine screen

*Annually from 13 years-15 years: hearing and vision screening, urine screen, teen screen

*16 years: Meningococcal vaccine #2, hearing and vision screening, urine screen, teen screen

*Annually from 17 years-18 years: hearing and vision screening, urine screen, teen screen

* girls who have started menses will get annual bloodtests to r/o anemia
